Please complete the enclosed forms and return them to me by Monday, August 6, 2001. If your child has had a physical examination within the past year it is not necessary to have another, however, you must submit the medical form found in your packet to your child's physician to be completed and returned to school. If your child has a physical during the year, please be sure to pick up a new medical form from school so that your child's record will be current. Office For Child Care Services also requires that all children be tested for lead poisoning and proof must be documented on his/her medical form. These forms must be on file at the school and available for O.C.C.S. inspection.
